To book an appointment as an established patient you'll need to fill out our intake form below. This details the information we need from you in order to book your next appointment. We are a fully electronic practice and all booking is handled via this submission form. 

Upon receipt of your answers, a team member will email you from our practice email, Please check your junk mail for receipt. This email will contain a list of our next earliest available time and dates to see you. Once you select your preferred appointment time and date you will receive an automated appointment confirmation. If you need specific availability, please indicate it in your preferences.

Appointment Time of Day Preference

Upon receipt of your answers, a team member will email you a list of our next earliest available time and dates to see you. Once you select your preferred appointment time and date you will receive an automated appointment confirmation. This appointment confirmation will reiterate your appointment date, location details, and check-in instructions.

Find us: We are at ground level, an entire corner retail space next to Perry’s Steakhouse, between Salzedo and Altara. Valet as of 11:00am and parking garages are located nearby.

It’s important to advise on same-day treatment because the block of time reserved for you will vary. Please know we have a 24-hour cancellation policy to preserve availability for all patients and respect patient time.

A valid credit card will be required to hold the reservation. Appointment changes should be sent to our practice email during practice weekdays Monday - Friday during practice hours 9:00am-5:00pm. Weekend communication is not monitored for scheduling purposes. Final confirmation of an appointment time/date means you agree to these terms.

We accept Care Credit, Credit Card, Debit Card, and Cash payment. Personal checks and banking apps are not accepted. Care Credit is not allowed to be used to hold your appointment. Photo ID must be presented upon check-out.

We ask you to reschedule your visit electronically if you feel unwell, or have been in contact with anyone that feels unwell. Cancellations and change fees may still apply. Per CDC guidelines, we recommend you continue to wear face coverings when in public areas, shared spaces, waiting rooms, and elevators. When necessary, this will be removed once you are isolated within the exam room and receiving treatment/consultation. Patients will also be asked to sign additional informed consent related to COVID-19.


IMO Cosmetic Dermatology holds the right to refuse treatment to patients who do not understand and agree to all informed consents.

Please know we are a fully electronic practice. DO NOT send medical follow-up questions or scheduling changes through social media once you become an established patient of our medical practice. All future scheduling changes should be redirected to the practice email on weekdays Monday - Friday during business hours 10:00am through 4:00pm. Please include your first and last name, DOB, and appointment date when communicating with us. We have hundreds of patients!

All patients are given provider contact information upon treatment. Please reserve provider contact information for emergencies only and redirect all non-emergent communication to the practice email at

We ask that you limit your visit to only yourself and do not bring any food/beverages, or excess of personal belongings to your appointment.

Questions related to treatment protocols, what options are best for you, and/or pricing depend on your in-person assessment and the extent of the potential acne/acne scarring, hyperpigmentation, volume loss, and/or muscle strength. Please do not submit questions about downtime, how to prepare for your appointment, after care, pricing, or product selection/education prior to your appointment. Advice related to this constitutes formal medical advice and should be reserved for your consultation. In order to further engage you regarding future availability beyond the aforementioned, we must receive your new patient intake information. Please confirm an appointment time and return intake information at your earliest convenience as appointments are first come first serve and we are currently working through accommodating increased requests to be seen.
